Beware of This Scam: Form 4022

Business owners may have received Form 4022 from the “United States Business Regulation Department” for “Mandatory Beneficial Ownership Report” filing. The form – which resembles an IRS tax form – states that businesses are required, under the Corporate Transparency Act (CTA), to complete the form and pay a $117 filing fee or be subject to penalties.

This is a fraud. Businesses should not fill out the form or pay the fee. There is no IRS Form 4022 and no filing fee under the CTA.
